TCM Physician -- Shingles Externally Healed But Pain Still Persists


Post-Herpetic Neuralgia (PHN) is a symptom  where sufferers experienced long lasting pain after the on-set of shingles had subsided.  The pain could be very acute and usually happened at night.  This could last as long as few months to two years.

 

Shingles is also known as Herpes Zoster.  It is a kind of skin disease that affects our nervous system.  It is known to be the reactivation of a viral infection (varicella zoster) within a nerve cell that causes a characteristic skin rash. Varicella zoster is the virus that also causes chickenpox. Normally, people who suffered from this disease will develop the feeling of tingling, burning sensation or sharp pain.  They normally will recover from eight days to several weeks. However, some people may be left with long lasting pain called Post-Herpetic  Neuralgia (PHN) even though the rashes had subsided.  The pain is left in the same area where the rashes had been. 

Post-Herpetic Neuralgia (PHN) occurs when damaged nerve fibers send confused and exaggerated messages of pain from one's skin to his brain.

From my clinical cases, most of these patients are elderly people who initially were not aware that they had contracted Shingles and they consulted Western doctor and took Western medicine only one week after the on-set of Shingles.

An 85 years old man came to see me and complained that he suffered from shingles for one month. He had seen a western doctor (GP) at the onset of shingles. He was prescribed amitriptyline HCL 10 mg, Codeine Phosphate 30 mg, "Augmentin" Amoxicillin trihydrate (Potassium clavulanate)".  He said now his shingles seemed to have gotten well externally. But he still has pain especially at night and needed to take painkiller. He said the pain was very disturbing during the night; it felt like ants were wriggling under the shingles. He had to take pain-killer every day. 

I told him that the discomfort and pain can be cured by acupuncture and herbal plastering. No burning of skin is necessary. Most people got well within a week (or less in some cases). He came every day for 6 days and gradually the pain and discomfort disappeared.

These kinds of experiences are told by many patients who suffered from shingles. Some of them (especially the elderly people) suffered for months (even years), not knowing what to do.

When I was working in Tan Tock Seng Hospital as an acupuncturist (over more than 10 years ago), an 80 years old woman was brought in by her daughter to see me. The woman had shingles on her private parts. Although her shingles appeared cured externally, she had pain every night. She said the pain was so bad that she could not wear her panty. When the thighs rubbed against one another, she would have pain and discomfort. So, she didn't want to wear panty. 

When she came to see me at Tan Tock Seng Hospital, she did not wear any panty. I gave her 7-8 sessions (roughly what I could recall) of acupuncture. Her discomfort and pain slowly went off.

At my own clinic now, I see many such cases every month and these patients are happy with the results. Normally, it takes 5 to 7 days to get well depending on the severity of the shingles. At the onset of shingles, we also give herbal medicine as it helps to cure faster.

Chinese traditional medicine (TCM) believes that besides virus attack, it is also due to poorer immunity and "heatyness" in the body.  So, we would advise patients not to take "heaty" and "toxic" food during the onset of shingles and also at least one month after the recovery of shingles.  

Shingles develop in the following stages:

1). Become red, painful and itchy, worse at night;

2). Next, form blisters with fluid or pus; colour turns transparent or yellow or green; still painful and itchy;

3). Gradually, it turns black and dried and forms scabs; 

4). Scabs drop off itself and healed; no more pain and itchiness. All these likely to take 6 to 7days to get well if shingles sufferer takes Chinese herbal medicine, apply herbal plastering and acupuncture.  Otherwise it will take longer.
