TCM Treatment for Gastric Pain and Irritable Bowel Syndrome in Singapore. Our TCM Clinic sees a lot of young adults with gastric problems, including Irritable Bowel Syndrome (IBS). Young people have a lot of social gatherings and they like to enjoy good food such as cold, spicy and fried food.  Some of them may take a liking to beer, liquor and wine.

A 69 year old lady had Gastric Pain

A 69 year old lady came to see me.  She said she had been suffering from gastric pain for the last two years.  She avoided eating spicy food because every time she ate curry or chili stuff, her stomach would feel uncomfortable.  If she ate too full at dinner time, she would have acid reflux, heart burn, esophagus/throat discomfort. She often burped and farted (emit gas) after meals.  Sometimes in the middle of the night, she would wake up because she felt gas rising gashing up to her throat. Her motion was regular but often had soft stool.  She was taking some Western medicine for sometimes. But her symptoms still remained unchanged.

I treated her with acupuncture and prescribed herbal medicine.  Two weeks later, she came back and told me that her symptoms had improved and she had less gas in her stomach and burped less often.  At night, she did not feel the gas coming up to her throat.  She requested for more herbal medicine.  I told her that she should avoid fried and spicy food during this time until she got well.  She came several times for acupuncture treatment and faithfully consumed my herbal medicine. Eventually her gastric problem subsided and she could eat anything she likes.

I have also treated many cases where these people felt "heaty" (上火) and often had headache, so they often took or drink cooling stuff such as watermelon, chrysanthemum tea "菊花茶", "金银花茶“, "24味凉茶“, 羚羊, bitter guard and coconut water to cool themselves.  After sometimes, they felt they often had stomach discomfort, gastric and bloatedness. These people who have stomach problem should avoid too much cooling stuff.

Many patients have gastric problem, gastric pain, bloatedness, flatulence, Irritable Bowel Syndrome (commonly known as IBS).  Many adults were suffering from Irritable Bowel Syndrome IBS for some time. Many of them have seen Western doctors for months or years and had taken western medicine.

Patients usually complained of stomach discomfort, flatulence, heat sensation or discomfort in their abdomen. In severe cases, the sufferers may feel that gas is rising gushing to their throat whenever they drink water or eat food. Those who suffer from Irritable Bowel Syndrome (IBS) told our TCM physicians that they often have to run to the toilet immediately after meals. There are some who feel stomach discomfort when they are hungry or feel bloated stomach pain after a meal. Through colonoscopy, some were diagnosed with having excessive acid in their stomachs. In some cases, they are diagnosed to have H.Pylori.

TCM (Traditional Chinese Medicine) physician adopts many ways to treat gastric problems, gastric pain. gastritis, flatulence and Irritable Bowel Syndrome IBS. We administer TCM acupuncture, expel air/wind using cupping method. We also prescribe TCM herbal medicine for the patients. At the same, we tell them to refrain from eating spicy and oily food, hard nuts (peanuts), alcohol, cold drinks, yellow noodle, chocolate etc. We advise them to take blander and easily digestible food. For those who have serious gastric problems, gastritis, stomach pain, flatulence, bloatedness, Irritable Bowel Syndrome IBS, we even advise them to drink mineral water during the occurrence period. We also tell them the cause of gastric problem is due to poor eating habits over the past years.

There was a case of a bank manager around 45 years old who told me that he had very bad gastric problem. He said he had been seeing his western doctor for 4~5 years but his condition did not get better. He came to see us TCM physician because his colleagues from China told him that he should see a TCM Chinese physician for his type of problem. They told him that TCM is good in treating chronic gastric problem. He told me that his stomach always had wind and he often burped and let off gas. He said that when he attended company meetings and needed to participate in the discussions, he felt that he could not speak too much as whenever he did so, he felt a gush of gas coming up to his throat and made him difficult to talk. He also told me that whenever he raised his voice to reprimand his staff for doing something wrong, he immediately felt that there was a gush of gas coming up from his abdomen to his throat. He said that he was very sensitive to a lot of food and felt very embarrassed whenever he was choosy on food when going out in a group for lunch. He came to see me and had two courses of acupuncture treatment with cupping (one course=10 sessions). He was also prescribed TCM herbal medicine. As his condition gradually improved, he faithfully underwent two courses. At the end of the treatment, he was happy to find his gastric problem cured.

We also have a few cases where the patients told me that they often felt a gush of gas coming up to their throat and often burp. Some of them told me that they burped non-stop. Strangely, when I did cupping and TCM acupuncture for them, they immediately stopped burping. I told them that they have to come back for a few more sessions for a long term cure.

For patients who suffer chronic gastric problem, I would analyze their conditions and ask them a few questions as follows:
1)   When they were hungry, do they have to find food immediately to ease their
       discomfort in their stomach?
2)   Do they have stomach pain after food or they cannot eat too full?
3)   Do they need to run to the toilet after food?
4)   Do they feel discomfort/gas rising gashing to their throat after food?
5)   Do they feel that constantly they feel something in their throat and could be rid off?
6)   Do they have heat or cooling sensation in the stomach area (left abdomen)?

We TCM physician will then prescribe them the appropriate TCM Chinese medicine and also give them a course of TCM acupuncture treatment. Through my personal experience, I found that those patients who came for one to two courses of acupuncture treatment together with oral TCM herbal medicine showed a great deal of improvement.

A HAWKER SELLING BEEF NOODLES HAD GASTRIC PAIN

A hawker at Sentosa was selling beef noodles (牛腩面) which was very popular. Every day many customers would line up to buy his noodles. He cooked beef noodles during peak hours including lunch hours. He felt very hot as he had to cook for many hours. In order to reduce his heat, he bought a small fridge and put many bottles of herbal cooling drink 白花蛇蛇草水 inside the fridge and he would drink them cold. He enjoyed the chilled drinks and it was effective in cool him down while he cooked. He continued to cook and drink the cooling water 白花蛇蛇草水religiously every day.

After approximately six months, he started to gastric pain. He ignored the pain and continued to drink the cooling water 白花蛇蛇草水every day. Gradually, he observed that his gastric pain got worse.

Every night, he would wake up in the middle of the night, and he felt the strong sensation of gas gushing up to the left and right sides of his chest causing him pain and discomfort. He would have to sit up for two to three hours to get relief from pain. Only after sitting up for hours, would he be able to lay down to sleep again. These continued for months, and his pain gradually became worse.

He visited our clinic for a consult to treat his pain. He complained about the pain and how intense it was which was disrupting his sleep every night. We administered acupuncture on his stomach and legs, and prescribed TCM medicine for him. The treatment brought on immediate relief, and he felt better after the first treatment. Thereafter, he would return to the clinic for treatment every week.

After taking our TCM medicine, he shared that he his gastritis was much better, and he was able to sleep through the night. He was grateful that the treatment and medication he received from was very effective in easing his pain.  He said that when he had stomach pain, he drank our medicine, soon after his pain subsided in about one hour, therefore he called our medicine 神仙药。

We advised him that he should stop drinking the cooling water 白花蛇草水, to allow his stomach to recover during this period.

In our expert opinion, drinking too much cooling water and too often worsen his gastritis.   We told him that can drink 白花蛇草水 or any other cooling water, but he must do it in moderation in the future.

A 63 yr old Caucasian Woman had Gastric Problem

A 63 yr old Caucasian woman came to see us for her gastric problem.  She said that for the last 9 years, she had been taking "Debridat" 100mg and "Dexilant" 60mg. She also took "enzyplex" whenever she had indigestion. She had her endoscopy examination on 22 June 2017. The histopathology report showed that she had chronic atrophic gastritis with extensive intestinal metaplasia and she had a few polyps in her stomach and colon, they were negative for malignancy.  However, her doctor said that it might be precancerous.
She also mentioned that her brother died of stomach cancer at the age of 53.  She said that if she ate spicy food she would get stomach ache.  Sometimes, she might experience acid re-flux or heart burn.  Therefore she was concerned and wanted to seek alternative treatment.

On 09 Dec 2017, I received an email from her. The content of her email was:

Dear Dr. Ong,

I just got back from my latest endoscopy exam.  Good news.  The doctor said he was surprised at the progress and improvement in the ulcers.  He's recommended to me that I end the Dexilant, as it's a very strong medication.  One side effect of the Dexilant is polyps in the stomach, and I assume intestines as well.  I have had several shown up in my stomach and colon now.  As a precaution, he removed a few and sent them to a lab for testing, I will know the results in about 3 weeks.

Personally, I think the improvement is a result of several factors.  Yes, the Dexilant helped with the acid decrease and probably helped the stomach to heal without all the acid present.  I also think the diet adjustment, despite when I couldn't always stick to it with my job, helped as well.  There may be some allergy issues I might have that helped contribute to not only the stomach problems, but also the torso rash.  Stress is an issue I constantly try to work on, which will help both problems of course.  And last but not least is the medication and advise you have provided.  As a believer in Eastern and Western medicine, I hope this combination will eventually cure my issues once and for all.

This a great "Christmas" present to me and I hope the improvement will continue into the next year.

Cheers,
Medeline Claire
